The single most common failure is a header row or a blank line left in the file — Blackboard's parser expects data to start on row one and treats any stray blank line as the end of the file, silently dropping everything after it.
The second is smart quotes. If you write or paste your questions from Word or Excel, those apps auto-convert straight quotes into curly "smart" quotes, which can corrupt the tab-delimited structure Blackboard expects. Writing or pasting into a plain-text editor first avoids this.
